Monthly Billing Statements are mailed at the beginning of each month.

Q How do I add an employee to our group dental plan?

To add an employee to your group dental plan, simply have the employee complete the appropriate enrollment form and then mail the completed enrollment form to Golden West

Q How do I remove an employee from my group dental plan?
A

Complete a Change of Status Form and then mail to Golden West. The Change of Status Form can be found in your administration kit. Remember, all terminations must be reported within 30 days of the termination date.
